Instructions:

1. LOTO / Assist Requests must be submitted 6-10 days prior to the event. Requester (PM, GC, or tradesmen) does Preliminary Scope, getting a firm understanding of the impact.

2. For multiple LOTO, an attachment LOTO Request Form is no longer required. Use "Multiple LOTO" option instead.

3. Provide your estimate for the dates and times of every tag out.

4. Once submitted, the notification will automatically be created and an email will be sent to the requester.

5. Planner converts Notification to Work Order.

6. Planner schedules LOTO Verification Walk with Campus Chief and Requester (providing the Requester the work order number).

6b. Chief decides if an MOP is required.

7. Planner or Chief confirms the LOTO Verification Walk Operation in SAP. The work execution is ready to be scheduled.

8. Planner negotiates and locks down LOTO dates with Requester and Chief one week before the scheduled execution.

9. Any changes to a scheduled operation must be communicated to the Planner as soon as possible (to allow for schedule adjustment).

10. "Emergency" LOTO requests must be justified on the "Emergent LOTO" notes section, below.

If Assist or LOTO request is less than 6 workdays from event date (Date of Action -or LOTO date), this form will automatically make this an "Emergency" and justification is required.

    When you bring additional fields into a conversion, Quickbase often finds inconsistencies. For example, say you're converting your Companies column into its own table. One company, Acme Corporation, has offices in New York, Dallas and Portland. So, when you add the City column to the conversion, Quickbase finds three different locations for Acme. A single value in the column you're converting can only match one value in any additional field. Quickbase needs you to clean up the extra cities before it can create your new table. To do so, you have one of two choices:

    • If you want to create three separate Acme records (Acme-New York, Acme-Dallas and Acme-Portland) click the Conform link at the top of the column.
    • If the dissimilar entries are mistakes (say Acme only has one office in New York and the other locations are data-entry errors) go back into your table and correct the inconsistencies—in this case, changing all locations to New York. Then try the conversion again.
